Please have your Singpass login details ready before proceeding.

Please note: Applications must be submitted using the applicant's personal Singpass account. 

Required Documents:
Please have the following documents ready to upload during the form submission: 

1. ACRA Bizfile of each Singapore company that the applicant is a controlling shareholder of (dated within the last 6 months)
   
   
2. Completed Business Projection form (one form for each Singapore company that the applicant is a controlling shareholder of)
   
   • Business Projection Form EN 
   • Business Projection Form CN 
3. Audited financial statements of each Singapore company that the applicant is a controlling shareholder of (for last 3 years)
   
   
4. CPF Employer Contribution Statements of each Singapore company that the applicant is a controlling shareholder of (for last 3 months)
   
   
5. MOM pass holder statements for each Singapore company where the applicant is a controlling shareholder
